Bromley PCNs

We are a collaboration of eight Primary Care Networks working together to provide leadership and innovation to deliver the best healthcare possible to local residents. Covering a total of 42 GP practices across Bromley, we are working hard to improve the way healthcare is delivered at scale to meet the needs of the local population, making the very best use of staff, space, technology, data insights and partnerships with local providers. We make sure that the patient is at the heart of everything we do 

Bromley PCNs are a partner of One Bromley, a Local Care Partnership which brings together Bromley organisations providing health, care and voluntary services to provide a more joined up, proactive and personalised care for Bromley residents. Each PCN is led by one, two or three Clinical Directors with the support of a Network Manager or Digital Transformation Lead and, in some cases, an administrative team, operating either remotely or at PCN premises.

Bromley GP Practices

There are 42 GP practices in the Borough of Bromley. Ranging from a patient list size of between 2,000 and 24,000, they each offer a range of appointment types, either in person, telephone consultation or online consultation, with a variety of clinicians between the core hours 8am and 6.30pm.

Bromley PCN teams have worked hard to develop the necessary ‘hard’ infrastructure – clinical IT systems, premises, equipment, interoperability, telephony – needed to deliver the PCN-wide services to patients and maximise staff capacity across member practices. Also key is the continuing development of PCN ‘soft’ infrastructure – staff training, management support and HR functions – to help recruit and retain a sustainable workforce.

PCNs are also now responsible for recruiting clinical and professional staff and running primary care clinics to help make more appointments available to patients. Together, we have made changes to improve access for patients, including the installation of modern telephony systems and a new patient online consultation tool, whilst making sure nobody is digitally excluded from healthcare. Numerous at-scale hubs have been developed to create more appointments across a multi-disciplinary team, with innovation at the heart of every new PCN project designed to reduce health inequalities and enhance proactive healthcare in Bromley.

PCN Services

Our objectives focus on embracing opportunities to use our local knowledge and expertise to develop projects and services that meet the specific needs of our patient population and to support the sustainability of general practices.  PCNs work with other healthcare providers to develop initiatives aimed at improving healthcare provision, for example by creating more appointments at hub clinics, bringing healthcare closer to home, making sure patients are not digitally excluded and working with hospital teams to reduce waiting times for children. Integrated Neighbourhood Teams

We are looking at ways to work better with other local organisations such as the council, hospitals, community services and the voluntary sector. To help us do this, Integrated Neighbourhood Teams will be developed so that everyone working on the health and wellbeing of our residents is doing so in a joined up way that makes a real difference.
