Remote Monitoring is a Bromley-wide programme to improve and intervene early in the care of patients most at risk of ill health. The service has helps to monitor patients at home who have certain long term conditions such as asthma, diabetes and hypertension without the patients needing to physically visit a GP practice.

In Bromley, BP@home was chosen as priority to begin remote monitoring activity as blood pressure control was poor amongst local patients.
All Bromley PCNs are delivering the BP@Home service to remotely manage cohorts of patients and encourage them to self-manage their blood pressure.
